COMPLETELY filled out, signed, and presented
at Registration. USE BLACK OR RED INK if emailing forms.

This is absolutely required for ALL students and their families to know AND follow them. Signing Registration Form acknowledges they will or have been read.

A prospective new student is welcome to take a FREE class to see if Paris Ballet and Dance is the right fit for them.
ASK ABOUT PARIS BALLET AMERICA, and ENSEMBLE ADVANCED TRAINING PROGRAM FOR SERIOUS STUDENTS WHO ARE CONSIDERING A CAREER IN DANCE.
